The new PS Form 1000, Domestic or International Claim, will replace all previous versions of PS Form 1000 and PS Form 2855, Claim for Indemnity — International Registered Mail, Insured, Ordinary Parcel and Express Mail, which are now obsolete. The Post Office employee section has been eliminated on PS Form 1000.

Customers may print PS Form 1000 from www.usps.com/insuranceclaims. Evidence of value is required and must accompany the PS Form 1000. Evidence of insurance must be retained by the customer until the claim is resolved.
