Located on Elm Street, our flagship location is an all-inclusive wellness center that provides a full schedule of activities, wellness programs, and opportunities to socialize and exercise.
San Diego PACE’s state-of-the-art facilities proudly serve 114 zip codes throughout San Diego. We encompass and provide life-changing medical care to the County’s South, Central, East, and North regions.
San Diego PACE offers easy access to care and closely monitors the health care needs of each participant which results in fewer health crises, ER visits, and hospitalizations, better management of medications, and improved quality of life.

FHCSD PACE is a Program of All-Inclusive Care for the Elderly that provides individualized care by an interdisciplinary care team (IDT) of medical professionals. The IDT is focused on working with older adult participants to establish care goals and address specific needs so older adults can continue to live safely at home.
