A form to complete which will allow the Fund Office to directly transfer funds from your HRA account to pay for your COBRA or self payment, or your retiree premium; this eliminates the need to pay first, then get reimbursed from your HRA account later.
Take this form to your financial institution and have them verify your entries and complete Section B. Return the completed form along with your Pension Application to the IBEW ® Pension Benefit Fund.
Please note that if you are changing beneficiaries you must complete 2 forms - the IBEW Pension Benefit Fund Beneficiary Designation Form and the Local 102 Fringe Benefit/Beneficiary Designation Form.
