4 sie 2015 · Learn how to calculate the correct dose of gentamicin and monitor patients to prevent toxicity. Find out the indications, side effects, body weight calculations and trough level monitoring of gentamicin.
This document provides dosing strategies and methods for aminoglycosides, including gentamicin, based on different indications and patient factors. It does not specify the gentamicin trough level, but suggests using the Hartford Nomogram or the Urban-Craig Nomogram for high-dose extended-interval therapy.
This document provides guidance on the prescription and therapeutic drug monitoring for once daily gentamicin therapy at Royal Cornwall Hospital Trust. It includes dosing table, trough level target, and adverse effects of gentamicin.
10 kwi 2023 · It is recommended to avoid prolonged peak plasma concentrations of gentamicin of more than 12 mcg/mL, and trough concentrations of more than 2 mcg/mL. Renal function should be evaluated twice-weekly in patients without previous renal disease through serum creatinine and blood urea nitrogen.
Trough levels are taken 18-24 hours after the dose was administered in a gold top tube - check this by hovering over the grey administration box on EPMA. Renal function should be measured during and shortly after completing therapy.
If the pre-dose (‘trough’) concentration is high, the interval between doses must be increased. If the post-dose (‘peak’) concentration is high, the dose must be decreased. For once daily dose regimens, consult local guidelines on serum concentration monitoring.
