That being said, each rank upgrade costs the same, there is no difference in upgrading an item from Rank 6 to Rank 7 or from Rank 11 to Rank 12. Neck, Cloak, Bracers, Rings, Shield and Offhand - Cost 250 Valor to upgrade.

The vendor is the same as for PvP gear, you earn points on m+ runs, 135 on all from +2 to +100000 and your gear is capped at 220. You only can upgrade "new" found gear and the cost depends on item type.
