Wheaton College Student Health Services. All students who are taking six or more credit hours at least one semester during the entire course of their study must show proof of immunity by returning this completed form BEFORE ENROLLMENT.
Copays are waived; deductibles apply. During the summer months, the visit, lab work, and procedure costs are charged to a student’s insurance. For medical advice after hours, a doctor-on-call is available at 508-285-9500 to address your concerns.

All incoming undergraduate students are required to complete the online course, AlcoholEdu® for College and Sexual Assault Prevention for Undergraduates®. You’ll receive instructions for completing these courses via email.
