Basically from what I can recall, Avery was the one that made Starved's story, and was also the one that added (or at least suggested, maybe) he be included in the Sonic.exe mod. Now obviously this probably isn't everything, so I suggest asking around so you can likely find a more definitive answer.

Sonic.EXE, is the central antagonist of Story Mode. He is a sadistic demon that resides in a haunted Sonic the Hedgehog cartridge. He is the main opponent of Too Slow, You Can't Run, Triple Trouble. He's also was an upcoming opponent of Final Escape and was planned to be a playable character in Slaybells before being replaced by Grimbo.

I'll say what I think. I think Tails was able to survive and defeat Starved after facing his fears, not unharmed, but not dead. Basically doing a SA1 moment where Tails saves Station Square and becomes a hero, just with nightmare fuel. Then he proceeded to be toxic and chat "ez" for hours.
