Achilles Tendon Rupture Non-surgical Rehabilitation Protocol. This protocol provides a guideline for rehabilitation of an Achilles tendon injury that is being treated WITHOUT surgery.
• Once non-operative treatment is deemed appropriate: ο Immobilization with cast in maximum passive plantar-flexion position and instructed to remain non-weight-bearing for 2 weeks. ο At two-week follow-up, patients are placed in Achilles-specific walking boot with 40° heel lifts.

Historically the tradeoffs were between a much higher rate of re-rupture of the tendon with nonoperative management (approximately 1 in 7 patients) versus the risk of wound complication with surgery. Weight-bearing with crutches and walking boot with heel lift to 30 degrees plantar flexion and 0 degrees dorsiflexion.
