The items carried by NPCs and in loot are now capped at a certain level appropriate to the character and location.

Having as few mods as possible also means you'll have an easier time using Oblivion Character Overhaul although there have been a number of Patches since it came out. That said, I'm playing the game completely vanilla these days (save for Darnified), and it holds up quite well.
