5 wrz 2023 · Tests measuring free T4 – either a free T4 (FT4) or free T4 index (FTI) – more accurately reflect how the thyroid gland is functioning when checked with a TSH. The finding of an elevated TSH and low FT4 or FTI indicates primary hypothyroidism due to disease in the thyroid gland.
The goal of this study was to evaluate different TSH cutoffs leading to reflex Free T4 testing, with the purpose to determine whether a widened normal range could decrease the need for additional Free T4 testing and not lead to missing cases of thyroid problems.
If the TSH level is low and the FT4 result is high this suggests an overactive thyroid (hyperthyroidism) that requires treatment. If the TSH level is slightly raised but the FT4 level is still within the normal reference range this is called subclinical hypothyroidism or mild thyroid failure.
