5 lip 2023 · As of July 5, 2023, nurse aides and applicants for nurse aide certification will apply via Texas Unified License Information Portal (TULIP), a web-based licensure system designed specifically for licensed long-term care providers and licensees. TULIP will allow Nurse Aides to complete all certification and application functions online.
To verify your Certified Nurse Aide (can) certification, use emr.dads.state.tx.us/DadsEMRWeb/. The Texas Nurse Aide Registry will return without action incomplete requests and requests without the required documents.
Search tips: For best results use Social Security number (SSN) or at least three letters of the last name. You may query by one or multiple social security numbers. You may upload a file from your computer to query by names in bulk format. Use the browse button to search for the file on your computer that you wish to upload for processing.
Submit an Application through TULIP: Log into the TULIP account to create an applicant registration, login and reset password. Select NA as license type, Complete a Name Change application. Documents tab: Upload a valid government issued ID and supporting documentation for name change such as a marriage license or divorce decree.
6 wrz 2023 · For expired licenses, navigate to the Nurse Aide (NA) licensure type in the application. Confirm that you meet the employment requirement for renewal and select "Renewal Application". Then click "Next" to view the application.
