5 wrz 2023 · Tests measuring free T4 – either a free T4 (FT4) or free T4 index (FTI) – more accurately reflect how the thyroid gland is functioning when checked with a TSH. The finding of an elevated TSH and low FT4 or FTI indicates primary hypothyroidism due to disease in the thyroid gland.
28 maj 2024 · Thyroid-stimulating hormone (TSH): High TSH levels are a sign of an underactive thyroid (hypothyroidism). They show that the pituitary gland is trying to stimulate the thyroid gland to produce thyroid hormones.
If the TSH level is low and the FT4 result is high this suggests an overactive thyroid (hyperthyroidism) that requires treatment. If the TSH level is slightly raised but the FT4 level is still within the normal reference range this is called subclinical hypothyroidism or mild thyroid failure.

Thyrotropin-releasing hormone (TRH) from the hypothalamus, thyroid-stimulating hormone (TSH) from the anterior pituitary gland, and T4 work in synchronous harmony to maintain proper feedback mechanisms and homeostasis.
Once inside the thyroid follicular cells, T3 and T4 are cleaved from thyroglobulin. Free T3 and T4 are then released into the bloodstream, where they are bound to serum proteins for transport. The primary transport protein is thyroxine-binding globulin (TBG), which has high affinity but low capacity for T3 and T4.
