File or Track a Claim. T-Mobile Premium Handset Protection, provided by Assurant, protects your device in the event of loss, theft, accidental damage (including water damage), and malfunction due to mechanical or electrical failure.
Online claim in your T-Mobile Account. Select File a claim or File damage claim Report lost or stolen. A details window appears providing the next steps. Ensure the mobile number is the correct one for the damaged, lost, or stolen device. If it isn't, return to the Accounts screen and choose the correct mobile number.
If your device was lost or stolen, call T-Mobile at 1-877-778-2106 to suspend your service and protect yourself against unauthorized charges. Have your mobile number and device information (manufacturer, model, and serial number). Your device's serial number (IMEI) is 15 digits long.

Device protection is not required to activate service with T-Mobile, but without protection, you may be responsible for the full replacement cost of your device if it is accidentally damaged, lost, stolen or suffers a mechanical breakdown.
