This document is a notification form for an employee to opt-out of or change their contribution amount to the SINDA fund. The employee provides their personal details and declares that they wish to either opt-out of contributions or fix their monthly contribution to a certain amount.

21 sty 2020 · The employer can stop the CDAC contribution of the employee immediately after the Opt-Out Form is sent out. A copy of the Opt-Out Form should be filed by the employer for record. No acknowledgement will be given by CDAC upon receipt of the Opt-Out Form.
