To be clear, this is the original path to my Sims 4 Mods directory ("directory" is a synonym for "folder"): "C:\Users\testpc\Documents\Electronic Arts\The Sims 4\Mods". Step 1 - create the new location: copy your Mods directory (or whichever directory you wish) to your desired location.
"Mods" will be at the same level as "Saves" in that particular "The Sims 4" folder. The Mods folder will appear in the Sims 4 folder in Documents, not the one in Program Files. Are you sure you don’t have a Sims 4 folder under Documents? Both the Documents one and the Program Files one are required for the game to work.

I would make sure that your Sims4 folder is where you want it, but make sure your Mods folder is inside your Sims4 folder. You should try and see if moving your MyDocuments>ElectronicArts>TheSims4 folder to the D drive lets you run the game.
