Treatment is directed at alleviating organ dysfunction, preventing irreversible scarring, and improving quality of life. Herein, we review the indications for treatment, pharmacotherapy, treatment duration, side effects, adjunct non-pharmacologic therapies, and outcomes for patients.

Except for facial nerve palsy, which may benefit from only a few weeks of therapy, long-term treatment is the rule, and most patients also receive second- and third-line therapy. MTX is traditionally the first second-line choice, with a response rate of about 60%, as seen in pulmonary sarcoidosis .
Sarcoidosis is the commonest interstitial lung disease (ILD), differing from most other ILDs in that many patients remain asymptomatic or improve spontaneously. Careful baseline assessment of disease distribution and severity is thus central to initial management.
