18 lip 2023 · This narrative review explores the physiology and evidence-based management of patients with severe acute respiratory distress syndrome (ARDS) and refractory hypoxemia, with a focus on mechanical ventilation, adjunctive therapies, and veno-venous extracorporeal membrane oxygenation (V-V ECMO).
“Open lung strategy” of low tidal volume with high PEEP and permissive hypercapnia is successful in the majority of ARDS patients. Life-threatening hypoxemia usually accounts for <15% of mortality in ARDS. Aggressive interventions are needed to manage critical hypoxia and minimize organ failure.
