The Rage Meter is a mechanic added in Revengeance Mode, along with the Adrenaline Meter. Once the meter has filled up completely, the Rage Mode buff can be activated by pressing the Rage Mode key. The Rage Meter has a minimum value of 0 and a maximum value of 100.

The Rage Meter is a mechanic in Revengeance Mode, along with the Adrenaline Meter (and Stamina Meter), that is unlocked by using the Metabolic Converter. Once the meter has filled up completely, the Rage Mode buff can be activated by pressing the Rage Mode key.
How about this: after defeating Polterghast, you gain the ability to merge your rage and adrenaline bars into unyielding rage. This version of rage has double the capacity of the original rage, will gain 5% rage for every hit you take and has the same damage bonus of adrenaline.
Revengeance Mode and Death Mode are difficulty settings intended to be much more challenging than Expert Mode, introducing mechanics such as the Rage Meter and Adrenaline Meter, and new boss mechanics.
Rage builds up by doing true melee damage (melee damage with melee weapons that lack projectiles, this is often specified) and taking damage from enemies. It's really only useful, then, for specialized true melee builds, so unless you're going for that you really don't need to worry about rage.
