It isn't perfect but it captures the feel of a MM stat block without requiring a program or image embedding. Obviously, the template is a conglomeration of many different abilities, but I wanted to provide the syntax/formatting for all the major components that could be in a block.
The best I've found so far is going to D&D Beyond's Encounter Builder, building an encounter that has each of the enemies they'll face, going into the Details page for each monster, printing to PDF, then pulling the PDF into photoshop to get rid of all the wasted space.
There's some Monster cards and Form fillable pdf's you can use to input the info, then print out on sheets of 3x3. They have a front and a back and are very simple to use (much like using pdf form fillable character sheets).
