Toxicity. Pyrimethamine is very safe and well tolerated. Occasionally megaloblastic anaemia, neutropenia, or thrombocytopenia may develop in patients with pre-existing folate deficiency.
Side effects of pyrimethamine include blood dyscrasias, rash and, very rarely, seizures or shock. At high doses, pyrimethamine causes bone marrow suppression, which can be prevented by concurrent administration of folinic acid (leucovorin).
