Annual Tax and Wage Report For Domestic Employment - DOL 4A (PDF, 225.15 KB) Annual tax and wage report which domestic employers must file. The annual report and any payment due must be filed on or before January 31st of the following year to be considered timely.This form is interactive and can be completed electronically and printed.
