The Crank Test is an orthopedic test to assess for SLAP lesions (superior labral tear from anterior to posterior) & proximal biceps tendinopathy. Gismervik et al. in the year 2017 performed a meta-analysis of shoulder tests for SLAP lesions and found that the Crank test had a sensitivity of 46% and a specificity of 72%.

The Crank Test is considered positive if the patient’s pain is reproduced. The test may also produce an audible or palpable clicking in the glenohumeral joint. This clicking should be repeatable.
The examiner should notice if the patient experiences relief of apprehension or just relief of pain in order to consider it positive. Several variations of this test have been described. In the “augmentation test” , the “crank test” and the “fulcrum test” an anterior force is applied in the position of maximum apprehension.
