Please contact your local police service to obtain a police record check. Photo identification that includes your name, date of birth and address is required. Acceptable forms of identification include drivers licence, photo health card, passport, birth certificate, Ontario photo card, permanent residence card, etc.

Completed record checks will be available electronically by secure download through your online account.
