Criminal history background checks for individuals are available at the PATCH site or download form SP 4-164, print the completed form, and mail it. An individual background check is $22 and can be notarized for an additional $5. Volunteer checks are free and can’t be notarized.

PSP established the Pennsylvania Access to Criminal History, or PATCH, for agencies, companies, and others to request criminal background checks on individuals. The information provided by the requestor will be checked against the criminal history database maintained by the Pennsylvania State Police Central Repository.

The Pennsylvania State Police established a web-based computer application called "Pennsylvania Access to Criminal History" or PATCH. Using this system, a requestor can apply for a criminal background check on an individual basis.
