There are many different definitions for what constitutes an incident or an accident, however the focus is always on unintended and often unforeseen events that cause unintended consequences. This article is focused on the process of learning from incidents and accidents.
Examples are: - Safety = freedom from unacceptable risk [2]; - Safety = the control of recognized hazards to achieve an acceptable level of risk [3]; - Safety = freedom from risk which is not tolerable. Safe = the state of being protected from recognised hazards that are likely to cause harm [4].
