2 mar 2020 · Renew your health card online using your valid driver’s licence or valid Ontario Photo Card as proof of identity and residency in Ontario. Your driver’s licence or Ontario Photo Card will not be renewed as part of this transaction.
To get, renew or replace your health card – including switching from a red-and-white card to a photo card – you will need three separate identification documents (Photocopies not accepted): an original one that proves your Canadian citizenship or OHIP-eligible immigration status.
Change of Information. Form used to change status of OHIP coverage - i.e., change of address, name, citizenship status, etc. or to cancel OHIP coverage or replacement of lost, stolen and damaged card.

To replace a red and white card, you will need to visit a ServiceOntario – Health Card Services– OHIP Office to re-register for a Photo Health Card. Please refer to the Ontario Health Insurance Coverage Document List (9998–82) for acceptable documents that can be presented.
