Log into your My Ontario account as a returning user. From your My Applications page, click on the file number link to begin your application. Your new application file number is different than your EOI file number.
Access the OINP e-Filing Portal and select ‘New Representative’ if you have not yet registered in the e-Filing Portal. Enter your last name, first name and email. You will be automatically redirected to log into your My Ontario account. If you don’t have a My Ontario account, click on ‘Sign up now!’
