You will need to use these forms when you file your case. Statewide approved forms are available for Adoptions, Appellate, Civil, Conservatorships, Criminal, Guardianships, Family Law, Juvenile, Name Change, Probate, Small Claims, and Traffic.
To access the Search for Case Number by Defendant Name in Criminal Cases and Civil Case Document Images, users are required to register with the LASC web site and create a user account.
