Ohio Home Care Waiver Providers. Providers are an essential part of Ohio Home Care Waiver. Medicaid relies on a wide variety of organizations and individuals to provide the services our consumers need to remain healthy and safe in their homes and communities.
Enrollment on the Ohio Home Care Waiver Program is available for Medicaid eligible individuals under the age of 60, who meet an Intermediate or Skilled Level of Care, and other program eligibility requirements.
Effective October 1, 2024, self-direction is available under the Ohio Home Care Waiver. Effective November 15, 2024, existing self-directed services are expanded under the MyCare Waiver. Use the navigation tiles below to find resources and information about self-direction.
