Health services are for students who need to take medicine (like insulin or albuterol) or receive a special nursing treatment at school. All forms can be found on the DOE Health Services webpage. You may also ask your school’s 504 Coordinator or nurse/medical professional for the forms.
The NYC Department of Education (DOE) and the Office of School Health (OSH) work together to provide health services to students with special health needs. If your child needs health services or medical accommodations under Section 504 of the Rehabilitation Act of 1973, complete the applicable form(s) in this packet.
Mental health resources are available to every public school in New York City.
