What is Northwell Link? Northwell’s EpicCare Link, Northwell Link, securely connects nurses, care teams, managers and providers (such as referring physicians) to a secure web portal. If someone you serve receives care at any Northwell location, Northwell Link gives you the tools to stay connected with their care.
In the Northwell Health app, you can connect to FollowMyHealth via your profile. The app can be downloaded to your phone from the App Store if you have an iPhone, or the Google Play Store if you have an Android.
