My account was restricted/locked due to the need to provide a proof of address at a local branch after the accounts opened were approved online. This issue was resolved the same day by going into the branch and providing an accepted proof of address.

TL;DR: Chase keeps locking me out of my account repeatedly and provides no information before or after they do so. If I call the number, they tell me to go to a branch. If I go to a branch, I get to access my account for a little while... until I get locked out again.
