To see information about your benefits (or report changes online), you will also need to fill in either your Case Number OR your 16-digit Wisconsin QUEST Card Number, OR your 10-digit Wisconsin Forward Card ID Number.
In the Payment Method section, select whether you’re paying with a new bank account or card account, and enter the account or card information. Choose whether the account is a checking or saving account. Choose whether the account is a personal or business account. Enter your bank routing number.
If you owe a monthly premium for your health care coverage through the Medicaid Purchase Plan (MAPP), follow the steps below to pay your premium using the MyACCESS mobile app. You cannot use ACCESS or MyACCESS to pay your initial premium. You must make your initial payment at your agency.
