Your request needs to be submitted in writing on a patient access form at a Quest Patient Service Center. If you need a form, you can download a PDF copy of the Patient Request to Access or to Disclose Protected Health Information (PHI) (Access Form) in English or Spanish.

You can get lab test results to your physician in several ways: • Print a copy and hand deliver or mail it. • Email. • Fax. • Send it in a Direct message if you have a Direct address and have set up a connection with your physician.
You can view the results in the MyQuest app or log in to questhealth.com and view the Quest Results page. You can also check the status of your results on the same web page. If you are waiting for lab results in California or Pennsylvania, your lab results may be held for a period of time before they are released.
