Go to the military police station to report a lost cac. They will give you a form to take to the cac people so they can make a mew cac. Not saying this is how it is ok every base, so take this with a grain of salt.
Replacing lost/stolen IDs is important and needs to be prompt. If you make soldiers run around for days getting signatures on pointless memos, that's how much longer the CAC is not invalidated.
Typically you fill out a lost ID report with SFS and you get a memo signed by the 1Sgt. The MPS may have the memo or the 1Sgt may already have it. Take those two with you to the MPS and get new ID issued.
