Before you fill out either form, please download and read the Hiring Hall Rules. Most areas on the forms are required, so if the form is failing to submit make sure you've filled it out completely. All information provided is confidential, only the hall has access to personal information and who is on the books.

Then print the completed form, sign and mail it to the Trust Office. These printable forms are in PDF format.
