Parenteral iron supplementation is preferable given its more reliable absorption, especially if iron deficiency is secondary to malabsorption. Parenteral supplementation includes use of iron dextran, iron gluconate and iron sucrose.

Iron therapy is recommended in all cats receiving ESAs. Iron status should ideally be assessed before starting treatment and thereafter on a monthly basis. In patients eating a commercial renal diet and where appetite is reasonable, vitamin deficiency should not be a concern.
Potential treatments for iron-deficiency anemia include oral or intravenous (IV) iron formulations. The IV iron formulations that are currently available in the United States include iron sucrose, low-molecular-weight iron dextran, ferrous gluconate, ferric carboxymaltose, iron isomaltoside, and ferumoxytol.
Patients intolerant of or who are refractory to oral iron, who have severe anaemia, who require urgent surgery or are more than 36 weeks pregnant may require treatment with intravenous iron. Treatment of iron deficiency should only very rarely be with a blood transfusion.
