Diplopia is very useful, as it will allow you to pick two Cube of Meat or Ball of Bandages instead of one when finding it, also preventing Conquest or Headless Horseman from ruining your chances of getting a Super Meat Boy/Super Bandage Girl with Book of Revelations.
Like Judas said: Dark One (one without bandages) only shoots brimstone downward. For Adversary (bandaged) I've found a good pattern: when he's on screen, circle around him as his homing brimstone seems to only fire directly up, down, left, or right (and doesn't home around him as you're circling).

Play as Lilith. Keep restarting until you get a ball of bandages on the first floor. Do not drop your spacebar item. Get to boss rush. Use spacebar item 3 times in boss rush.
This is easy to get now that Ball of Bandages is in Greed mode. All you need to do is hold R for a minute or two as Lilith and then you've got it.
Ball of Bandages blocks enemy shots. Each subsequent Ball of Bandages obtained are combined with the first one (increasing its 'level'), changing its characteristics: Level 2: Gives the orbital the ability to shoot Charm tears that deal 3.5 damage. It shoots once every second. It shoots once every 0.73 seconds.
Some of the most difficult achievements in the game are the "no damage" achievements - Basement Boy, Spelunker Boy, Dark Boy, and Mamas Boy. They are unlocked by completing The Basement, The Caves, The Depths or The Womb without taking damage, respectively.
