Welcome to the online application for services available through the The Bureau of Disabilities Services (BDS). This application is used to apply for waiver services and/or group home placement. If you are currently receiving BDS services, please contact your local BDS district office.
To apply for BDS services there are two options available. The BDS Gateway is an online application available 24/7 where individuals and families seeking may submit an application for services. The BDS Gateway may be accessed at: Bddsgateway.fssa.in.gov.
