IMRF provides two types of disability benefits: Temporary disability benefits. Total and Permanent disability benefits. You always start out receiving temporary disability.
How to apply. If you will be disabled for more than 30 consecutive days, you should apply for disability benefits, unless your employer will pay you for your entire disability period. We recommend you complete a "Member's Application for Disability Benefits" form (Available in Member Access).
To be eligible for disability benefits you must have: At least 12 consecutive months of IMRF service credit since being enrolled in IMRF. Service credit in each of the 12 months immediately preceding the date of disability. Have 12 consecutive months of service credit prior to the gap in service.
Form 5.42 is available online at www.imrf.org. Your physician(s) must send the completed form to IMRF along with copies of your medical records from the date of your disability. • Write down the name(s) of the person you spoke with and the date. • Ask when the physician(s) will send the form to IMRF.
Please use this checklist when applying for IMRF disability benefits. You can obtain IMRF Forms 5.40 and 5.42 by calling 1-800-ASK-IMRF (275-4673) or by downloading the forms from www.imrf.org. As soon as you stop working and you feel you will be disabled more than 30 days. . .
File an Application for Disability Benefits (IMRF Form 5.40) with IMRF. We recommend you submit this application online through your Member Access account at www.imrf.org/myimrf. You can also fax your application to IMRF at 630-706-4289. File the form even if you filed a worker’s compensation claim.
IMRF Information and Resources. The program provides its members (employees within local units of government) with retirement, disability, and death benefits. The benefits for IMRF members are divided into tiers based on the time period at which an individual becomes a member of the program.
