Welcome to the Illinois State Police Online Crash Reporting Application. This program has been designed to allow motorists involved in certain types of vehicle crashes to make a report online without the on-scene assistance of a law enforcement officer.
The Crash Forms Request page provides law enforcement agencies with an online form for ordering the paper SR 1050, Illinois Traffic Crash Report forms and related materials. To use the form:
Obtain a Crash Report by Mail. All requests for crash reports will be governed by the Illinois Freedom of Information Act (FOIA), 5 ILCS 140/1, and you will be receiving a redacted version of the requested records.
