1 kwi 2020 · It is also important to note that altered response to opiates may significantly affect the adverse effect profile experienced by the older population. Common disease states in older adults play a significant role in producing these adverse effects if the wrong opioid is chosen.
The side effect profile of opiates is similar for all age groups; however the elderly population is at a greater risk for these side effects given their comorbidities and high incidence of polypharmacy.
Another side effect of high-dose opioids is respiratory depression, which is more common in elderly people. This effect is mediated by the μ-opioid receptor, to which agonists such as morphine and fentanyl have a distinct dose-dependent effect . Morphine, oxycodone, hydromorphone, fentanyl, and methadone all cause a dose-dependent reduction in ...
16 mar 2018 · TABLE 3 lists the common side effects of opioids and suggestions for mitigation. Patients and their caregivers should be informed about the expected benefits of opioids. Opioids reduce short-term pain, and complete pain relief may be unlikely with long-term use.
In another study, side effects happened with a rising dose or duration, including agitation (48%), cognitive dysfunction (39%), myoclonus (20%), and tremor (20%) . Oxycodone is a semi-synthetic opioid with parallel clinical efficiency with morphine. It has fewer adverse events, such as nausea, vomiting, and confusion .
Several opioids should not be used in older adults due to ADEs. These include meperidine (neurotoxicity from active metabolites), pentazocine (psychotomimetic effects), butorphanol and nalbuphine (sedation, confusion), and propoxyphene (cardiotoxicity and prolonged QTc); the latter has been removed from the U.S. market. 16
1 cze 2016 · To avoid adverse effects, opioids for persistent pain in an older adult should be prescribed at the lowest possible dose that provides adequate analgesia. Due to age-related changes, finding the best treatments may be a challenge, and understanding the pharmacokinetic implications in this population is key ( Table 2 ).
