Fracture of the proximal humerus is a common injury often seen in older patients and one of the true osteoporotic fractures. Proximal humerus fracture (PHF) includes all fractures to the anatomical neck, surgical neck, greater tuberosity (GT), or lesser tuberosity, either isolated or in combination.

Diagnosis is made with orthogonal radiographs of the humerus. Treatment can be nonoperative or operative depending on location of fracture, fracture morphology, and association with other ipsilateral injuries.
Objectives. Understand the anatomy and surgical approaches to the humeral shaft. Understand the indications for nonoperative vs operative management of humeral shaft fractures. Understand the use of functional bracing in humeral shaft fractures. Understand the literature comparing ORIF vs IMN.
