Diagnosis is made with orthogonal radiographs of the humerus. Treatment can be nonoperative or operative depending on location of fracture, fracture morphology, and association with other ipsilateral injuries.

This study attempts to quantify the longer-term utility of reducing the risk of humeral shaft nonunion, by offering early surgery to all patients. At five years post-injury, the estimated cost-effectiveness of routine humeral shaft fracture fixation fell below the NICE threshold of £20,000 per QALY gained.

1 wrz 2022 · Functional bracing, with its low cost and satisfactory clinical outcomes, is often the most cost-effective strategy for humeral shaft fracture management. ORIF becomes preferable at the higher willingness-to-pay threshold when societal burden is considered.
Among patients with closed humeral shaft fracture, internal fixation surgery, compared with nonoperative functional bracing, did not significantly improve functional outcomes at 12 months. However, the substantial amount of treatment crossover from nonoperative to surgical treatment should be considered when interpreting the trial results.

At injury, the upper extremity is stabilized with an above elbow hanging cast or a coaptation splint, spanning from the lateral shoulder, turning 180° around the elbow, and ending just distal to the axilla.
