A simplified SNAP application/recertification form is now available for households where every person is age 60 or older, or has a disability, and no one in the household has earned income. You can download the simplified form below or request an application/recertification kit be mailed to you by calling the DSS OneNumber at 718-557-1399.
5 sie 2022 · The SNAP Periodic Report Form asks SNAP recipients to inform HRA of any changes to their case. The report is due every six months after you start receiving or renew your benefits. If you do not submit it within six months, HRA will close your case.
If you live in New York City and you are not also applying for Temporary Assistance, you can apply on-line at https://www.nyc.gov/hra, or call or visit any SNAP Office and ask for an application package.
20 wrz 2024 · The SNAP office will mail you a recertification packet for you to complete and mail back to HRA. You can also recertify online on ACCESS HRA. Once you submit your recertification, call 718-SNAP-Now (718-762-7669) for an interview. Apply online.
You may also complete your recertification application using the Supplemental Nutrition Assistance Program (SNAP) Application/Recertification (LDSS-4826) and report this change during the interview with your HRA worker.
