The Indiana Department of Health Division of Vital Records is responsible for maintaining and issuing certified copies of vital records, including birth, death, and fetal death certificates for events that occurred in Indiana.
To order a death certificate by mail in Indiana, print and fill out the application form, and mail it (along with the fee or payment information) to: Indiana State Department of Health. Vital Records Division. 2 North Meridian Street. Indianapolis, IN 46204. The application form is available from Indiana Department of Health.
