Paper size refers to the printed size of your artwork/document. We can print in the following default sizes: Loose Sheets. A7, A6, A5, A4, A3, B5, Half Letter, Letter, Small Card (55 x 85 mm), DL, 105/148/200/210 mm Square. Postcards. A7, A6, A5, Half Letter. Folded Sheets, Encapsulated Sheets & Synthetic Sheets.
To get the size we should have in pixels, we simply multiply the size of the thing we are printing in inches, with the desired DPI resolution. 2.48" x 3.45" at 300 dpi is (2.48 x 300) = 744 pixels and (3.45 x 300) = 1038 pixels. MPC says we should add 1/8" extra bleed area on each side of our card, and they approximate that to 36px at 300dpi.
