The first step in the HOME Choice enrollment process is completing the program application. Click the arrow below for the HOME Choice online application. Gather the following necessary applicant information prior to beginning the online application.
To qualify for this program, you must be at least 18 years of age, have needs that can safely be met in a home or community-based setting as determined by the Ohio Department of Medicaid or its designee, and meet each of the additional four requirements listed below .
Ohio’s HOME Choice program transitions eligible Ohioans from institutional settings to home and community-based settings, where they receive services and supports at home and in their communities. Who is eligible to participate? Be enrolled in Medicaid.
There are three ways to apply for Medicaid benefits or renew your benefits. Your Medicaid benefits will need to be renewed annually and you will be notified when it is time to renew. Just like the application process, you can apply/renew online, in person, by mail, or on the phone. Read the renewal form carefully.
A child born to a woman with full Medicaid coverage on the date of the child’s birth is automatically eligible for Medicaid. The newborn is deemed eligible based on the mother’s Medicaid coverage.

On the application, NC Medicaid is referred to as “Medical Assistance.” Apply Online Through ePASS. You will need an NCID to begin your application. An NCID is a secure online user ID and password you set up to access many North Carolina government programs, including NC Medicaid.
