Employees Withholding Allowance Certificate G-4 (PDF, 555.49 KB) Form (G4) is to be completed and submitted to your employer in order to have tax withheld from your wages.
By correctly completing this form, you can adjust the amount of tax withheld to meet your tax liability. Failure to submit a properly completed Form G-4 will result in your employer withholding tax as though you are single with zero allowances.
The G4 form is a government document used to file a tax return with the Internal Revenue Service (IRS). To find a blank copy of the G4 form, you can visit the IRS website at https://www.irs.gov/forms-instructions and search for “G4” in the forms library.
Georgia Form G-4 is the state’s Employee’s Withholding Allowance Certificate. This form is available from the Georgia Department of Revenue (GA-DOR). Its purpose is to inform a Georgia-based employer of the amount to withhold from an employee’s paycheck for Georgia’s Individual Income Tax.
INSTRUCTIONS. The Law requires you to complete Form G-4 so that your employer can withhold Georgia Income Tax from your pay. (Form G-4 remains in effect until changed or until February 15 of next year if "Exempt" is claimed on line 7.)
