Fluid therapy is not only meant to compensate intraoperative losses but should also take into account those occurring prior to surgery, induced by poor water intake, bowel preparation, major inflammation associated with a stress response, and possibly, hemorrhage.
In summary, fluid therapy remains a contentious area of perioperative medicine, with differing approaches seen. It is vital that patients receive fluids targeted to their individual physiology. When a fluid challenge is given it should be an effective challenge of the patient's physiology.
